You can decide the first number very easily but in order to decide to second and the third numbers, you may have some experience so the only thing you can do is to predict the results. If you know how to use Photoshop, you may also try finding the result by mixing colors using different layers. Be sure to reduce the opacity of second and third layers.

When it comes to deciding the best color, it all comes down to your imagination. Can you image yourself having that hair color? If you can’t, you can also use Photoshop but changing hair color in Photoshop is little tricky so you might need help while doing that.
If your skin tone is cool, you should prefer cooler tones and if your skin tone is warm, you should prefer warmer tones. If you don’t know your skin tone, there is also a small test that might help you.
Vein Test: If your veins are blue, you should prefer cooler tones. If your veins look green, you should prefer warmer tones.
Jewelry Test: What suits you most? Gold or silver? If your answer is silver, then you should prefer cooler tones. If your answer is gold, you should prefer warmer tones.
Most people re-dye their hair 2 weeks later using the same color to get the best results. I can’t say how much you should wait but 2-4 weeks usually works fine for most people so I should suggest the same.
